Apolipoprotein E (ApoE) is a polymorphic protein that serves as one of the most crucial lipid transport proteins in the human body. It is also the apolipoprotein with the greatest impact on lipid metabolism, primarily involving triglycerides and cholesterol. ApoE is distributed throughout most tissues in the human body and is mainly found in chylomicrons (CM), very low-density lipoproteins (VLDL), intermediate-density lipoproteins (IDL), and some high-density lipoproteins (HDL).
ApoE4, with arginine residues at positions 112 and 158, exhibits altered lipid-binding properties and a propensity to form neurotoxic oligomers. It is the primary genetic risk factor for late-onset Alzheimer’s disease, present in approximately 15–20% of the population. ApoE4 impairs amyloid-β clearance, promotes tau hyperphosphorylation, and disrupts synaptic plasticity. It also exacerbates neuroinflammation and mitochondrial dysfunction in astrocytes.
